National Healthcare Decisions Day 2016 Event
ELC is pleased and honored to host a very important, free, and accessible community event on April 13, 2016 to celebrate this year’s National Healthcare Decisions Day.

According to the “National Healthcare Decisions Day” website (www.nhdd.org), “National Healthcare Decisions Day exists to inspire, educate, and empower the public and providers about the importance for advance care planning.” The attorneys and staff at the ELC are extremely passionate about the importance of advance planning for healthcare decision-making for anyone over age 18. It is out of this passion and strong belief that all competent persons over age 18 should have access to the information necessary to understand Durable Powers of Attorney for Healthcare, as well as the tools necessary to create one for themselves, that the ELC is hosting this complimentary, community event.

The ELC event will be held on  April 13, 2016, at our located at 2111 Plum St., Suite 201, Aurora, IL from 10:00 a.m. to 3:00 p.m. Members of the public are invited to schedule a complimentary appointment during those hours to obtain information about Illinois’ statutory Durable Power of Attorney for Healthcare, and if they wish, to receive assistance in creating one for themselves that day. Although walk-ins are welcome, walk-ins will only be accommodated if time permits and will be handled on a “first come/first serve” basis. Renzi notes that attendees, who wish to create a Power of Attorney for Healthcare while at the event, must bring with them the names, addresses, and all telephone numbers for all of the persons that they wish to name as their agent, and as their successor agents. The ELC encourages persons, if possible, to designate at least two back-up (successor) agents to the person they are naming as their original agent. Scheduling an appointment is strongly encouraged. For more information about this event, or to schedule an appointment during the event hours, call the ELC at 630-844-0065 (ext. 109; or, ext. 123).
